To assist in making it easier and faster for you to sign up for our programs we have listed each Aquatics & Safety program and the required forms below.  The forms are also available from the front desk of the Student Recreation Center. 

Filled out forms MUST be brought to the front desk of the Student Recreation Center.  They cannot be emailed or faxed.  Once the forms have been turned in and payment has been made, you will be officially enrolled in your desired program.
